After getting the form, fill up with necessary information and attach necessary Documents like a photocopy of HKID / Passport to be certified (*) by a branch manager of a bank, Justice of the Peace, certified public accountant, lawyer or notary public and a copy of residential address proof dated within the last 3 months (e.g. a utility bill, a bank statement or credit card statement) and return them by mail at given address and your account will be ready for you in a few days.
