Select Facility/Ancillary, (Facility/Ancillary services include but are not limited to: Dialysis Services, Durable Medical Equipment, Home Health, Laboratory Services & Prosthetics. You may be responsible for copayments or coinsurance.) such as Urgent Care Centers, Convenience Care Clinic, Mammography Facilities, MRI/CT Facilities, Labs, Colonoscopy Facilities, and Other Facility types.

You can minimize your search by entering provider’s first name and last name, language spoken and selecting gender such as no preference, female, and male.
